The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

It is our mission to provide you with affordable, high-quality treatment that fits your budget! For your convenience we accept Visa and MasterCard payment options, and also offer financing for cosmetic services. Cosmetic procedures require payment at the time of service. We thoroughly explain your options, so that we can accommodate your needs.

Insurance

Remember that insurance policies vary, and we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. We participate with most health insurance (HMO and PPO programs), including:
