Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.

Payment and Financing Policy

As manner of payment we accept cash, personal checks, Mastercard, or VISA. There is a $35 charge added for checks returned due to insufficient funds or checks that are cancelled prior to deposit. Full payment for cosmetic services, or any of our cosmetic products, is due upon check-out at the completion of your visit.

For clinical appointments, copays are always collected in advance when checking in for your visit. Please be prepared to pay your copay, as prescribed by your insurance company, when you arrive. After your insurance claim is processed, you will be billed for any remaining deductible or out-of-pocket portions. Payment for these charges is due upon receipt of your first statement.

OUT-OF-POCKET: Should you receive a letter from your insurance company notifying you that you’ve reached your out-of-pocket limit for the year; we ask that you bring a copy to us at your next appointment so we can waive the copay.

COLLECTION: Failure to pay charges in a timely manner may result in your account being turned over to a collection agency. If this occurs, interest and fees will also be added to the balance as required.

BUDGET PLAN: In these days of high copays and high deductibles, we offer an interest-free budget program for the out-of-pocket portion of your insurance plan. Please don’t hesitate to ask us about a budget plan if you are not able to pay your balance in full. Please call our business office at 301-668-6134 to make payment arrangements.

Appointment Scheduling and Cancellation Policy

Please call our office at 301-668-3004, Monday through Friday from 8:30 to 4:30, if you wish to schedule an appointment. Also please know that you can call our office anytime, day or night, to cancel or reschedule an appointment.  If you call us outside normal business hours, just leave a message – we’ll call you back on the next business day.

Every patient receives an automated phone verification call 3 days before their scheduled appointment. For surgical appointments and Total Body Skin Examination appointments, these automated calls are followed up with a personal call by our appointment desk staff. We ask that you please confirm or cancel your appointment by responding to these phone calls. Otherwise, if you need to cancel an appointment, we kindly request that you call out appointment desk at least 24-hours in advance of your appointment. It is normally our policy to charge a $30.00 fee for missed appointments when we don’t receive notification of the cancellation (exceptions are made for emergencies of course).  We wish to remind all of our patients that access to health care is important to everyone, and any prior notice we receive about a cancellation allows us to offer that time-slot to another patient who may benefit from it. Even as little as 24-hour notice can make a difference. Patients with multiple missed appointments also may be discharged from our practice.

In the event you are running late, please call our office. If you are more than 15 minutes late for your scheduled appointment you may be asked to reschedule.

Insurance

We ask that you please check with our office or your insurance company to confirm that we are a participating provider on your health insurance plan and verify your benefits in order to minimize any out of pocket costs. If we are not a participating provider on your plan, you will be expected to pay for services provided at the time they are rendered.

IMPORTANT
HMO'S (as noted above) and Medicare HMO replacements require a referral from one's primary care provider (PCP).

Elizabeth Liotta, MD Dermatology strives to maintain a current list of our in-network insurance companies. Due to the ever changing nature of individual insurance companies, the above list may not be all inclusive.

Prescription Drug Refill Policy

The fastest way to get a prescription drug refill is to have your pharmacy fax a Prescription Drug Refill Request to our dedicated prescription fax line at 301-668-6135. Refill requests that are received in this manner can usually be returned to the pharmacy within 24 hours (on business days). If there are no refills remaining on your prescription, please call us at 301-668-3004, select Option 3, and speak with our Patient Care Coordinator or leave a message. Refill requests received in this manner are usually acted on within 24 to 48 hours (on business days). Please note that in some cases you may need to schedule a follow-up appointment to get a new prescription.
